I understand that I am responsible for all setup & cleanup of the facilties for my event. Rooms will be available 30 minutes before and after the scheduled start and end times for setup and cleanup (if additional time is needed please note that in the comments).

Requests for the use of Saint Anne's facilities for private events require separate approval, paperwork and payment. Parishoners requesting private events will be contacted by Saint Anne's staff to complete their request once this form is submitted.
