The parish has installed a keycode lock on the east entrance door to the Eternal Adoration Chapel and choir loft stairway. Parishioners may request an access code for 24/7 access to the Chapel by submitting this form; only one form is required per registered family.

Access codes may be changed periodically for safety and security. Do not share your code with anyone unless they have been approved by the pastor or maintenance supervisor. In addition to the new lock system, the parish has also installed security cameras to provide a higher level of security for the church and safety for the parishioners.
 
As an access code holder, you are responsible for anyone who enters the church with you, as well as anyone who uses your code to access the church.
 
Please read the following list of guidelines. By signing below, you acknowledge these regulations and assume full responsibility during your use.
 
Eternal Adoration Chapel Users: 

• Unlock the door using your code and ensure it latches closed behind you; keep all interior doors closed when not in use.
• Never touch the monstrance directly. If you are arriving and there are no other adorers present, reverently and carefully open the screen around the monstrance’s glass case to open it to view. If you are departing and there are no other adorers present, reverently and carefully close the screen around the monstrance’s glass case to close it from view.
• The restroom may be used as needed by adorers only, it also serves as a locking safe-haven within the chapel if at any time an adorer feels uncomfortable with regard to their safety.
• Do not rearrange furniture or place any items on the altar.
• If you find the chapel or any part of the facility out of order, notify the pastor and maintenance supervisor immediately.

Music Ministry Users: 

• Unlock the door using your code and ensure it latches closed behind you; keep all interior doors closed when not in use.
• Out of respect for the Blessed Sacrament, always maintain silence in the choir stairway.
• Keep choir loft and music director office doors closed at all times and locked when not in use.
• Only use the restrooms in the church narthex; the chapel restroom is reserved for adorers. Ensure the door from the choir stairway to the narthex remains closed and locked when not in use.
